How to view your personal medical history and records

This article explains how patients can access and manage their personal health information in MediSprout. Learn where to find provider notes, view or update your medical history, and access documents shared by you or your provider—all securely from the My Records section.

Patients have full access to their personal medical histories and documents that have been gathered while using MediSprout, including all visit records and all sharable provider notes. 

Here’s how to access your personal medical history and records. 

Sign in to your MediSprout account on your app or laptop. From the main menu, select
My records. In the submenu that appears, select Provider notes to see any visit or other note created by your provider. 

Select Medical history to see the details of your personal medical history form that you completed when you onboarded with your provider. If you haven’t completed your medical history or need to update the form, you can do so now or at any time. Be sure to Save your edits. 

Lastly, select Documents to see all documents you might have previously uploaded, or to see documents uploaded by your provider. 

All information held in My records is confidential and available only to you and to any medical provider with whom you are connected.
