Setting up your superbill information

A superbill is a document your provider generates so you can submit it to your insurance company for reimbursement of out-of-pocket visit costs. For your superbills to be accurate and accepted by your insurance, MediSprout needs a few pieces of information from you in advance.

What information appears on your superbill

  • Your legal name and date of birth, as they appear on your insurance card
  • Your home address
  • Your insurance company name, member ID, group number, and the policyholder's name and date of birth (if the policy is not yours)
  • Optional but recommended: your driver's license or another government-issued ID for identity verification

How to set up your superbill information

  1. From the main menu, select Profile.
  2. In the Insurance section, select Add insurance.
  3. Enter your insurance company name, member ID, and group number. If the policy is not in your name, select The policyholder is someone else and add their name, date of birth, and relationship to you.
  4. Upload a photo of the front and back of your insurance card so your provider's billing team can confirm the details.
  5. In the Identification section, add your driver's license. See How to add your driver's license to your personal information.
  6. Confirm your home address is current under Contact information.
  7. Select Save.

When your superbill is generated

After each visit, your provider generates a superbill if you have requested one or if it is standard for the practice. You will receive an email and an in-app notification when it is ready. From the main menu, select Billing and then Superbills to view, download, or share the document with your insurance.
