How to use notes templates
Notes templates speed up clinical documentation by giving providers a structured starting point. MediSprout ships with industry-standard templates and lets administrators create custom templates for the organization.
Templates available out of the box
- SOAP — Subjective, Objective, Assessment, Plan.
- DAP — Data, Assessment, Plan.
- GIRP — Goal, Intervention, Response, Plan. Added in Release #40, connected through the same template API used by AI Notes / HealthScribe transcription, so it stays in sync across web and mobile.
- Free-form — no structure, write whatever you need.
- Custom templates — anything your administrator has published.
How to apply a template to a note
- During or after a visit, open the Note tab.
- Select a template from the Template dropdown.
- Each section of the template appears as a heading with a prompt. Fill in each section.
- Save the note. You can change template later — your existing content is preserved and moved to the closest matching section.
How to create a custom template (administrators)
- From the main menu, select Settings → Note templates.
- Select New template and give it a name (e.g., "Pediatric follow-up").
- Define each section: section heading, optional prompt text, and optional default content.
- Add as many sections as needed and drag to reorder.
- Save and publish. The template appears in the Template dropdown for all providers in your organization.
Editing or retiring a template
Administrators can edit templates at any time. Edits do not retroactively change notes that have already been saved with the older template version. Retiring a template removes it from the dropdown but does not delete notes that used it.
