How to use notes templates
Note templates give providers a structured starting point for clinical documentation, reducing the time spent writing notes and helping ensure consistent records across the organization. MediSprout includes two standard clinical templates — SOAP and DAP — plus a free-form option and any custom templates your administrator has published.

Templates included by default
Every MediSprout account includes the following templates out of the box:
- SOAP — Subjective, Objective, Assessment, Plan. The most widely used clinical note format.
- DAP — Data, Assessment, Plan. Common in behavioral health settings.
- Free form — No predefined structure. Write whatever you need without section prompts.
- Custom templates — Any templates your administrator has created and published for your organization.
How to apply a template to a note
- During or after a visit, open the Note tab.
- Select a template from the Template menu.
- Each section of the template appears as a heading with optional guide text. Fill in each section.
- Save the note.

Note: You can switch templates after you've already started writing. Existing content is preserved and moved to the closest matching section in the new template. Nothing is lost when switching. |
Copy from a previous patient note
To speed up documentation, you can copy the content of a patient's most recent note into a new note. This is useful for follow-up visits where much of the clinical context carries over.
How to copy from a previous note
- Open a new note for the patient.
- Select the option to copy from their last note.
- Review and edit the copied content before saving or finalizing.

Note: The original note is not affected. Only the content is copied — the new note remains fully editable and independent. |

Pending notes in Past Visits
The Past Visits list now highlights appointments that have incomplete (pending) notes. This makes it easy to spot and complete any overdue documentation at a glance — you no longer need to open each visit individually to check note status.

How to create a custom template (Administrators)
- From the main menu, select Settings → Note Templates.
- Select New template and give it a name (for example, "Pediatric Follow-up").
- Define each section: heading, optional guide text, and optional default content.
- Add as many sections as needed and drag to reorder them.
- Save and publish. The template immediately appears in the Template menu for all providers in your organization.
Edit or retire a template (Administrators)
Administrators can edit a published template at any time. Edits do not retroactively change notes that were already saved using a previous version of the template.
To retire a template, open it in Settings → Note Templates and select Retire. Retiring removes the template from the Template menu but does not delete any notes that were written using it.
