How to view your provider notes

MediSprout gives you secure access to the notes your provider has shared with you after each visit. This article explains where to find those notes, what you will see, and what to do if you have questions about them.

Where to find your provider notes

On the web (browser)

  1. Sign in to your MediSprout account at app.medisprout.com.
  2. From the main menu, select My Records.
  3. Select Provider Notes.
  4. Your list of available notes appears, organized by visit date. Select any note to open and read it.

 

On the MyMedi Health app (iPhone or Android)

  1. Open the MyMedi Health app and sign in.
  2. From the bottom navigation bar, tap My Records.
  3. Tap Provider Notes.
  4. Tap any note to view its full content.

What you will see

Provider notes appear once your provider has finalized and shared them. Not all notes are automatically shared — your provider controls which notes are made available to you. Notes you can typically expect to find include:

  • Visit summaries and progress notes from completed appointments.
  • Any notes your provider has explicitly chosen to share with you as part of your care.

 

Each note shows the visit date, the provider's name, and the note content. You can read the full note but cannot edit it.

 

Notes from AI-assisted visits

Some providers use AI Notes — a feature that helps generate clinical notes from visit audio. If your provider uses this feature, it only activates on your visits if you have given your consent.

 

To check or update your AI consent: go to My Information → Privacy and select View consent form. You can grant or withdraw consent at any time.

 

Granting AI consent does not change which notes are shared with you. It only controls whether AI may assist your provider during note creation. Your provider still reviews and finalizes every note before it is saved.

 

Accessing your other medical records

My Records contains more than just provider notes. From the same section you can also access:

  • Medical History — the personal health form you completed when you joined your provider's practice. You can update it at any time.
  • Documents — all documents you have uploaded, plus any documents or external links your provider has shared with you.

 

Privacy and security

All information in My Records is confidential. It is only visible to you and to the providers you are connected with in MediSprout. MediSprout does not sell or share your clinical data with third parties. All data is handled under HIPAA-compliant security standards.

 

If you have questions about a note

If something in a note is unclear or you believe it contains an error, contact your provider directly through MediSprout:

  1. From the main menu, select Messages.
  2. Start a new message to your provider and describe your question or concern.

 

Your provider will be notified and can respond through the platform. For urgent medical concerns, call your provider's office directly.
